   2. A detailed listing of Current Scholarships and/or Grant(s) per semester and when they end as well as other
       scholarships being applied for as outlined in Section IV above.
   3. TWO (2) OFFICIAL COMPLETE COLLEGE TRANSCRIPTS.  Duplicate copies not accepted.
   4. A copy of the first two pages of your parents'  2006 IRS Form 1040.
   5. Contact information on TWO (2) personal references in Houston - include both phone #s and addresses.
       List YOUR RELATIONSHIP to your references.
   6. Personal letters of recommendation from Both:

       These letters should be in a sealed envelope with the writer's signature across that seal.   They should focus on
       leadership, campus and chapter involvement, ambition and goals, and personal character.
 
 7. A Personal Letter from Applicant

  8. If you are taking an Internship, a detailed explanation must accompany your application. (Credit hours, working hours,
      salaried, unsalaried, etc.)
  9. Copy of your Official High School Transcript to verify Harris Count residence.

PLEASE NOTE:

Any change in your projected academic/sorority plans after this application is submitted will require you to re-qualify
 to receive the award. You do not have to reapply just inform us of the change in plans and we will determine if you still
 qualify.
